Incomplete documentation in all APS working tools to ease follow-up.

Low engagement in the pediatric surge by APS. Hence weekly tracking for elicitation and testing of all children and siblings will be monitored.
Backdate of Insertion of all ICT forms into the patient file starting on the 1st. by all APS with respective cohorts.
Systematic use of ICT forms during elicitation.
- Individual APS registers crosscheck for proper documentation. and where gap, immediate update by concerned.
- Emphasis on the weekly plan of work for site per program area.
- Distant monthly triangulation with CHUY before online validation on the 1st with APSs.
- Report triangulation and online uploading.
