After review of the PMTCT cohort monitoring register, alots of gaps were identified from offering of services to proper documentation. Women who missed appointment were found in the tracking register and VL not systematically done to pregnant and breastfeeding women. She was shown how to document follow up visits in the cohort monitoring register and VL documentation. A line list of women eligible for Vl was established together the aps and she asked to call up these women to come for sample collection. Results not found in the regsiter, she was asked to document all VL results in all the reporting tools ( Pateint files, Registers). Also update the CCM register for PMTCT cohort which was not updated. On the spot mentoring was done the follow up of this cohort
